Age Connects Torfaen is pleased to deliver a Carers Post Discharge Service. This service is a free service, available for Carers who are either being discharged from the hospital or their loved one is being discharged. This service covers all 5 main hospital sites. Support is provided for up to 6 weeks, with a maximum of 5 hours support available per week. Service Operational Hours The service will be available from 9.00-4pm, with our last call ending at 7pm The service is available Monday – Friday List of services available for up to 6 weeks: • Shopping service – (client will pay for the shopping) • Cleaning service ( only light duties, no deep cleans) • Accompany to follow up appointments • Home sitting service • Prescription pick up • Meal delivery service (first meal free) • Health & beauty services ( hair dressing or low level toe nail cutting) • Full benefits assessment or benefits review • Application for blue badge • Assist with preparation of light meal/ snack • Onward referral to other support services
